How often does an office need it?

Annual. Risk-based duty: review regularly and whenever there is reason to believe it is no longer valid (no fixed statutory interval; 12 months shown as a recommended review cadence). Since 1 Oct 2023 the assessment must be recorded in full regardless of premises size, with fire-safety arrangements and the assessor's details (Fire Safety (England) Regulations 2022). FOR LANDLORDS of buildings with 2+ dwellings the FRA covers the structure, external walls (incl. cladding/balconies) and flat entrance doors (Fire Safety Act 2021); 11m+ buildings add quarterly communal fire-door checks and annual best-endeavours flat-entrance-door checks, and residents get fire-safety instructions annually (Fire Safety (England) Regulations 2022).

What happens if you don’t?

Enforced under the RRFSO 2005. Minor breaches: fines up to GBP 5,000 (magistrates'). Serious breaches that put people at risk of death or serious injury: on indictment an unlimited fine and/or up to 2 years' imprisonment; prohibition notices can force closure.
